* Include your child's name, age division and team name. Please also include whether you would like

your child's full name, first initial and last name or no name at all. Our photographers will specifically

get action shots of your child and select the best one for your child's sign. You will be emailed when

your yard sign is ready to be picked up at the tent during the season.
